Typically, the receipt from your latest vet visit will list all due date reminders for your pup's vaccines. 

Before uploading, please check that Bordetella, Distemper/Parvo (often listed as DHPP or DHLPP) and Rabies are all included on your sheet, and that they all expire AFTER your stay with us. Thank you!
